OVERVIEW
The wisdom of learning from failure is incontrovertible. Yet organisations that do it well are extraordinarily rare.
In these changing, fast paced times, learning our lessons is more important than it has ever been before. Can we really afford the time to repeat our mistakes again and again? The truth is that if we do not learn our lessons we will be left behind. We need to consciously and mindfully develop the skills, qualities and processes and culture that allow Lessons to be identified and learned.
"We've GOT to stop constantly re-inventing the wheel here!" or “If we do what we always do, we will get what we have always got” Agreed?
Why? First of all, we too rarely look at Lessons Learned in terms of how and through whom, value will be produced. Secondly, all too often the lack of value-creation by Lessons is because the regular actions and habits that create or leverage learning are not encouraged by the culture in which we work, the behaviours we regularly see around us and are usually not in any job description–in fact, in many settings it is actively discouraged by existing structures and norms, however unintentionally.
Too many Lessons Learned efforts engage people in producing what are essentially recommendations for (often unspecified) others. Learning is a dynamic and fast paced complicated process. The only way in which we will successfully create a Lessons Learned culture, is through a deliberate and planned approach, with explicit intentions. We cannot leave the Lessons Learned processes to chance. We need to prioritise and place a sense of urgency on true honest learning and upon identifying the root causes, and consistently applying the necessary corrective actions to ensure that we do not repeat avoidable, expensive, sometimes life and death mistakes, again and again. On the other side of the same coin, we also need to ensure that we learn from our successes so that we are clear about what we need to repeat as best practice.
In order to truly address the Lessons Learned challenge and create the environment in which this is possible, we need the values and behaviours to ensure that people feel safe to be open and honest and to contribute for the benefit of the greater good, the business improvement, the individuals affected, or often a life or death scenario or incident. If we are to succeed in changing ingrained habits of defensiveness or fear and encourage humans to be openly involved in Lessons Learned processes, we need to be aware of the human psychology that is impacting on our behaviours. Without the right values and culture, we cannot create the learning environment in which true, successful and continual Lessons Learned can prosper and lead our behaviours.
Therefore, we need skilled people to drive the lessons and hear people’s stories, challenges, issues and successes. We need people to capture these stories and to take responsibility to process the learning and the lessons so that the learning is understood and shared and the new corrective and successful behaviours are known, practised and believed in.
Key outcomes of the Programme
- This programme focuses on the traits and the qualities required to become an effective and Agile Lessons Learned Practitioner in order to create a positive learning environment and culture within the organisation
- Learn how to create a positive and non- threatening learning environment where Lessons Learned conversations can lead to positive application of the corrective actions without defensiveness or fear.
- The use of a specifically designed Lessons Learned Psychometric Tool is an optional addition to support individual and team development.

As part of a wider culture change project, becoming a Lessons Learned Practitioner is the second step of the process. Please note that this programme can also be taken as a stand-alone.
Here are the three steps so you can see the wider context:
Step 1 -The first step is to have a foundational programme which introduces the whole concept and culture of Lessons Learned. This is for all people so that the necessary foundation and awareness raising is established.
Step 2 -To train Lessons Learned Practitioners who have the skills in facilitating meetings and workshops and to create the Learning environment necessary to hear people’s stories, issues, challenges and successes
Step 3-Here we train Lessons Learned Ambassadors in order to have people who have the role and responsibility to encourage the Lessons Learned culture and to be the agent for change that stimulates open and honest conversation.
